Enhanced Care Management is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of high cost and/or high-need Medi-Cal recipients through services and comprehensive care that is community-based, interdisciplinary and person-centered.

The Community Outreach and Engagement Specialist is responsible for acting as bridge between the community, Sanctuary Centers’ Enhanced Care Management (ECM) program and other community service providers. The Community Outreach and Engagement Specialist engages in direct recruitment of clients and raises awareness of the services provided by ECM program. The person possesses good communication and interpersonal skills along with strong organizational skills.

The Community Outreach & Engagement Specialist reports to the Program Director of the ECM Program.

Essential Duties and Responsibilities

  • Collaborates with ECM team members, enrollees and community partners
  • Develops and sustains positive relationships with a range of people, including enrollees, community leaders, local groups and organizations, and local authorities
  • Performs face-to-face outreach, and online activities with ECM enrollees
  • Maintains case notes and participant files and ensure that all reporting requirements are adhered to as outlined by Sanctuary Centers and ECM program for every client interaction
  • Work collaboratively with community partners and other Sanctuary Centers staff in linking clients with needed services
  • Attends meetings and works as part of the ECM project team
  • Willing to work in an office-based environment and out in the community
  • Willing to work some evenings and weekends
  • Performs other duties/special projects as assigned

  • Understand, maintain, and uphold the mission and vision of Sanctuary Centers of Santa Barbara.

Education and Experience

  • Bachelor’s degree required
  • Bilingual, both English and Spanish required
  • Extensive experience working in community outreach and engagement, particularly with disadvantaged groups
  • Ability to manage a varied and challenging workload
  • Strong written and verbal communication skills required
  • Strong computer skills are required especially with case management data systems
  • Must be hard working and reliable professional willing to be a role model.
  • Must possess a valid driver’s license and motor vehicle insurance to operate a motor vehicle in the State of California
  • Must pass a criminal background check

Job Type: Full-time

Pay: $26 – $29 per hour DOE
